The Web App will be embedded into the home page of Faxnow.

I think there may be some misunderstanding regarding how this Web app is going to be used.   The design needs to be more compact because it will reside in a placeholder and will not be a stand alone webpage.   You will note that the theme I am using is narrow.  It can be viewed on a mobile phone also.   The white space we discussed between the field names and the fillable fields will probably not be an issue if the changes I list below are completed.
